My company has asked me to fill out a survey that they will be using to find a different medical insurance plan. The survey contains all of my identification, including social security, as well as several pointed questions about medical history (e.g., AIDS, chest pains, history of cancer, etc). This seemed slightly questionable to me and I was hoping someone could way in with some legal guidelines on what an employer can and cannot do.

A Kaiser website shows that New Hampshire requires guanteed issue of medical insurance for “small groups,” and defines small groups as “1 to 50.” However, the same website says that New Hampshire has no guraranteed issue for individuals. I must be missing something. Isn’t an individual a group of “1″?

I just called my doctor to set up an annual exam and the woman on the phone said the office was “tier two” with my insurance company. What does this mean? Will I have to pay out of pocket?
