What do these medical insurance things mean?

For a form, I have to fill some information for my medical insurance. I do not understand what the Medical Insurance Provider, policy/group #, address of insurance co., DOB of policy holder, name of policy holder.

    name of insurance company, your policy number with them found on your ID card, address of your insurance company usually found on the ID card, your date of birth, your name if you have the insurance.

    the medical insurance provider is the name of your insurance. ( ex. unitedhealthcare or blue cross blue shield) the policy group number can be found on your insurance card. they are labled as such. on the back of the card it will tell you the address of the insurance company where you can send claims. and the policy holder is the persons name who holds the policy. (ex. i am the policy holder and my daughter is a dependent) and then the dob is also for the policy holder. hope this helps.

    all of that information should be able to be obtained from you Health Insurance card.
    Medical Insurance Provider: who will the claims be submitted to? examples: United Health Care, Blue Cross Blue Shield.

    Policy/group# should be listed on the front of your insurance card.

    Insurance company phone and address should be listed on the back.

    DOB (date of birth) of policy holder: if you are the primary person – it is your date of birth. If you are covered under a different person’s policy (like Mom or Dad or Spouse) then it is THEIR date of birth.

    name of policy holder: who ever is the primary person on the policy. either you, or the name of the person who’s policy you are an additional insured for.

    Policy holder is the person who holds the policy (so you, your parents possibly, spouse). Medical Insurance Provider is the name of your insurance company (Blue Cross Blue Shield, Aetna, etc). Policy number is your ID number/subscriber number-it should be on your card. Group number may also be on your card–if not, you can ask your insurance company. Address is the address to send claims to–should be on the back of your card (if not, you can call the insurance company).
